When using the following forms, highlight the form you wish to copy, and paste it into a document that you can print on your computer. We are hoping to have these in pdf format soon! for your convenience!

Local After-Care Information: this handout can be printed two on one page to save paper. This handout contains the names and phone number of the aftercare personnel to contact if an animal fixed at the clinic experiences problems after surgery. This form should be printed on brightly colored paper so the owner will not lose it!!

Veterinary After-care Questionnaire

Thank you for your generous support of the MSNTF mission by providing post-surgical care for the many patients attended to at the event in your community. In the interest of providing veterinary care in future MSNTF events, we try to follow up on any post-surgical problems that you might encounter in your role as the follow-up veterinarian. The easiest way to do this is by providing you with the following form to fill out at your convenience.

We know you are very busy, again, thank you for your time and support of the MSNTF and its mission. Feel free to make copies of this form if needed.

*Problems include, but are not limited to: Bleeding, torn sutures, licking, abnormal swelling, reactions to suture material (seroma), and infection.

Your pet had MAJOR SURGERY TODAY!!!!
This is not to be taken lightly.
YOU ARE RESPONSIBLE FOR AFTERCARE!!!

  • Your pet NEEDS TO BE KEPT WARM AND QUIET.   Isolate your pet in a dry, warm, quiet place where it will not be allowed to play or jump. 
  • Your pet CANNOT GO OUTSIDE WITHOUT AN ADULT AND ON A LEASH for 7 to 10 days after the surgery.
  • Your pet CANNOT LICK AT THE INCISION.  This will cause infection.  Wrap a towel (see attached instructions) around your pets neck positioned so that they cannot lick or if you have a cone, please put it on the animal. 
  • KEEP YOUR PET AWAY FROM CHILDREN AND OTHER ANIMALS.  Your pet cannot play, jump or run because stitches could give way, causing severe pain and additional surgery for your pet
  • Your pet CANNOT GET WET FOR 7 TO 10 DAYS AFTER SURGERY.
  • Your pet WILL NOT NEED TO HAVE SUTURES REMOVED.  They will dissolve. HOWEVER, if you feel the sutures are not dissolving, please call the aftercare number.
  • If you need to pick your pet up, put your arms around its bottom and around its chest.  DO NOT PICK UP THE ANIMAL BY ITS FRONT LEGS OR SHOULDERS.
  • The day of surgery, give your pet SMALL PORTIONS OF SOFT FOOD AND FRESH WATER. Feeding or watering your pet excessively may cause vomiting and/or diarrhea. Your pet should eat and drink within the first 24 hours. 
  • DO NOT give any type of pain medication including Tylenol, Aspirin, or Ibuprofen.

If you feel there is a problem with your pet, PLEASE call one of the numbers on the aftercare handout AS SOON AS POSSIBLE!
